EuroHealthNet organised a workshop “Connecting primary care and prevention for integrated service delivery” at the 23 European Social Services conference “Making Connections - Improving Lives: Building partnerships for better outcomes” in Lisbon.  The workshop explored the preventive role integrated care can also have in reducing pressure on social protection systems, decreasing the need of long-term care and preventing conditions from worsening. Two presenters from the European Forum for Primary Care presented the integration of care in Portugal and the Family Health Units in Portugal.  The workshop gathered primary care practitioners, local authorities, social services managers, policy makers, and social workers with examples from the audience in England, Bruges, Campania and Sardinia. EuroHealthNet referred to work from previous projects that show how EU Structural Funds can be used for reducing health inequalities (www.fundsforhealth.eu )
