The European Forum for Primary Care (EFPC) 2025 Conference will focus on enhancing collaboration across healthcare teams within primary care, with the theme of “Playing the Symphony of Interprofessional Primary Care: Harmonizing Research and Practice”.
The conference will address three main perspectives on interprofessional primary care:

Professional/team perspective: Exploring effective models of interprofessional collaboration, leadership in teams, and the role of interprofessional education.

Health system perspective: Addressing community-oriented primary care, policy innovations, impact assessment models, and digital tools like AI to support collaboration.

Patient/citizen perspective: Emphasising people-centered care, patient involvement in decision-making, and addressing special needs, including healthy aging and palliative care.
